Registration is ongoing

July 13th – July 31st has been designated as a time during the summer that returning students can be registered if they have not been registered at a cost of $75 per family and $10 for each additional new student (for returning families only)

Documents needed for New Registration

Kindergarten
Parents or guardian must present an original or state-certified copy of the child’s birth certificate. The child must be five years old (5) on or before September 1, 2009

1st Grade
Parents or guardian must present an original or state-certified copy of the child’s birth certificate. The child must be six years old (6) on or before September 1, 2009

2nd – 12th Grades
Must present a copy of the birth certificate or proof of guardianship and:
Social Security Card
Certificate of Immunization (GA form 3231- GA-DHR)
Certificate of Ear, Eye, and Dental Examination (GA form 3300 – GA-DHR)
Photo ID of the parent or guardian enrolling the child
Proof of Residency documents
Discipline Report (middle and high schools) for transfer students
Withdrawal records from previous school including last grade placement

Students returning to the school must present the following for registration

Parents must update the Mohammed Schools registration form and any school-related materials each year [field trip permission slips, dismissal, after-school arrangements, medical forms, etc…]
